The Comments & Tasks section is giving me an error from each of the collaborators that have commented stating “The shape was deleted”, and the Add Comment icon is missing from the flow. I’m unable to determine where their comment was made within the flow chart. I tried reverting back to a previous version, but the problem still persists. How do I recover the comments?

When you delete an object that has a comment, the comment becomes orphaned in the current version. To get both the object and its comment back together, you must restore (or create a new document from) a revision from before the deletion occurred. Restoring a version taken after the shape was deleted will not recover the comment.
