Could anyone recommend any specific template(s) to use for creating a theory of change diagram.

Hi, Michael.
“Theory of Change” and “Logic Model” mean slightly different things. I have never needed a Theory of Change diagram (they don’t have set templates), but I have used Logic Models.
I strongly recommend swimlanes (although in the example below, I hid them). They not only track different dimensions over categories / phases, but (if you do not get too fancy) allow cross-linking. Here’s an example, used in getting a grant for https://docs.fakenewsfitness.org:
Here is what it would look like from a full swimlane example for a project I’m involved with, Resilient Greenfield, using SIPOC (which was crazy overwhelming so I did not decide to use it):
For different varieties, within Lucid, search the templates for “Swimlanes”. Hope this helps!
