Hello Rob thanks for posting in the Community! You can link these forms to flow chart stages by clicking on the shape or object you would like to be linked to the form locating the lightening bolt at the top of the page and choosing whether you want to link it to a web link a different page within your document a different Lucidchart document in general an email address or add a layer.
