Hi @Luc P, I recommend taking a look at the UI Mockup shape library in Lucidchart. It has several components of menus and forms that you could combine to achieve something very similar to this!

The button bar and selection dropdown in particular would help you recreate the form shown above. 

As far as a connector goes, I recommend using a basic line and customizing the endpoint with this menu to indicate the relationship you want to show.
